Participate in the Victorian Department of Health survey on COVID-19 vaccination for 5 to 11 year olds

The Victorian Department of Health has launched the first state-wide survey of parent and guardian attitudes and perceptions towards COVID-19 vaccination for children aged 5 to 11 in Victoria.  

  The data gathered in this survey will enhance the pathways and resources that support parents and guardians to make decisions about COVID-19 vaccines and vaccination services for 5 to 11 year olds. 

Who should complete the survey? 

All parents and guardians with 5 to 11 year old children are encouraged to complete the survey, 

regardless of whether their children have or have not been vaccinated.   

What is involved? 

The survey will take 5 to 7 minutes to complete and is open until 5pm on 17 March 2022.

Completion of the survey is voluntary. The survey is entirely confidential, and all data will be handled following the Department of Health Privacy Statement.
