Before your child’s stay
Paperwork
Please ensure that upon arriving at Monash Children’s Hospital you have brought with you the following completed documents:
- Any paper work that has been sent to you for completion from Monash Children’s Hospital
- A list of your child’s medical history outlining any significant illnesses and injuries and/or special requirements
- A list of current medications
What to bring
- Baby dummy, teddy and/or favourite toy
- Charged mobile phone and charger
- Nappies
- Change of clothes
- Pyjamas
- Bottle of milk/formula (if bottle fed)
- Medicare or private health insurance card
- Current medications
